Application forms for Disclosures can be obtained by any of the above methods. If you are required by a potential employer or voluntary organisation to undertake an Enhanced or Standard Disclosure, then the employer or organisation (that is, the Registered Body) may also be able to provide you with an application form and guidance notes on how to complete the form.
Anyone requiring a Basic Disclosure can also complete an application online. Further information and the online application form can be found at BDO. Online completion of Enhanced and Standard level disclosures is NOT available at present.
