How do I obtain Registration and Disclosure application forms?
We have tried to make arrangements for access to Disclosure application forms as flexible as possible. Stocks may be held by employers/voluntary organisations, etc. (these may be handed out to applicants at the time they are offered the position). Applications are also available by telephoning/faxing/emailing or by writing to Disclosure Scotland.
You can also apply for a Basic Disclosure online via this website. Go to BDO. Please note that completion of Standard and Enhanced applications is not currently available online.
